The horizontal mattress suture is a strong everting stitch that can help close wounds under moderate tension. It is placed in a way that the needle enters and exits on one side of the wound, then crosses to the opposite side, creating a horizontal pattern. This stitch is useful when stronger wound edge support is needed and when slight eversion of the skin edges is desired.

This technique is often chosen for wounds where simple interrupted sutures may not provide enough support. It can help distribute tension more widely across the wound edges. However, if tied too tightly, it may compromise blood supply and leave pressure marks on the skin, so careful tension control is important.
